Eevee 40 lv and Thunder/Fire/Water Stone (not 30 lv) -> Jolteon/Flareon/Vaporeon
Vulpix 50 lv and Fire Stone (not 40 lv) -> Ninetales
Zubat 30 lv (not 40 lv) -> Golbat
Oddish 20 lv (not 18 lv) -> Gloom
Gloom 40 lv and Leaf Stone (not 50 lv) -> Vileplume
Venonat 40 lv (not 50 lv) -> Venomoth
Don't remember about Psyduck, but I think it was 40 lv required to evolve him into Golduck.

The same Cubone. Required lv to evolve him in my opinion is 40 or 50 lv.
Rhyhorn 60 lv (not 65 lv) -> Rhydon
